I have a friend, Anil, that is an MD and is currently traveling internationally before he starts his residency. In December, he got back from a 4-5 month trip to Central and South America.
For this trip, he flew into Johannesburg, South Africa on Valentine's Day. He's traveling towards Tanzania where he will work as a doctor. Then he's traveling to who-knows-where afterwards. (I think he's trying to out do my trip this summer.)

This is also the friend that traveled around with me for two weeks during my European backpacking trip in 2006.
